Dear Patients,
I am writing this letter with mixed emotions. Although I had planned to remain
in private practice for many years, it has become necessary for me to close
Crestani Family Medicine, effective January 4,2007. I know many of you will be
disappointed. Although I will miss the relationships I have developed with you,
I have made arrangements for most of you to receive your care through a
wonderful physician, Dr. Elisa Haley at Work Well, Live Well. She has agreed to
accept responsibility for charts and to provide care for you and your family. I
have known Dr. Haley for many years, first meeting her when she did a rotation
with me as a medical student. She graduated from the same family medicine
residency program as I did (UAB in Huntsville) and practices medicine in a
manner similar to mine. I feel she will provide excellent care for you. Mary and
Kim from my office will be working with Dr. Haley as well, and all patient
charts will be transferred to Dr. Haley. For those of you with Medicare,
Medicaid or Tricare, Dr. Haley is currently unable to accept you as patients,
but will provide you a copy of your chart upon your request. The Tricare and
Medicaid offices will help to reassign those of you insured by these companies.
The Madison County Medical Society has a list of physicians currently accepting
Medicare patients.
Whether you will be going to Dr. Haley or to another physician, I encourage you
to call and schedule an appointment as soon as you are able. Please, take a few
moments now to check your prescriptions and see when the refills will expire or
when you will run out of medications, or when you need your annual exam. Even if
this is 6 months away, call now and get an appointment before that time. When
you call, tell the receptionist that you have been a patient of mine. Please be
considerate of Dr. Haley as she absorbs the 3000 patients you all represent.
I have truly enjoyed all the time I have spent in private practice, with each of
you, and your families. While this will be a change for us, I am confident that
this is in the best of interest of all. It has truly been the hardest of my
life, but I have felt the presence of the Lord each step of the way. Know that
you will receive the care you deserve and that I will continue to pray for you.

FOR PRESCRIPTION REFILLS, APPOINTMENTS AND MEDICAL RECORDS CALL:
Medicaid and Tricare patients call the respective insurance office to be
reassigned.
Medicare patients call the Madison County Medical Society (881-7321) for a list
of physicians accepting new patients.
For all other patients, please call Dr. Elisa Haley at 830-8930
Dr. Haley will have all the patient records. You do not need to get your
records if you will be seeing Dr. Haley; Medicare, Medicaid and Tricare patients
may call her office to request records.
No patients will be seen at Crestani Family Medicine after noon, January 4,
2007.
